Pay: $1,854 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: ASAP About the Position TRAVEL - SOCIAL WORKER - CASE MANAGEMENT (917) - Swedish MC - Days Unit Information: (Case Management/917) Location: Address: 501 E Hampden Ave Floor: Multiple Building: CM office at 601 E Hampden Ave 260 General Information Tell us about the unit(s): Number of Beds Units are mainly 20 beds each. Number of Staff Team of CM/SW to manage load Type of staff: Day Shift Friday-Monday 7:530 Day Shift Monday Friday 8-4:30 Night Shift - Patient Ratios SW: 1:40 RN: 1:20 Type of equipment mobile, computer EMR Meditech/Midas/Navihealth Typical hiring profile Skill Set Most recruited for: (Travel RNs, local CNAs and sitters) Must have: candidates without these skills will not be considered for the role. Behavioral Health experience with suicide risk and substance use assessments Discharge planning Preferred or nice to have: candidates with these skills will be considered first. ED experience would be helpful but not required Palliative Care Knowledge and GOC Conversation List typical procedures performed on unit(s): Suicide Risk Assessments SBIRT/Substance Assessment Discharge Planning Assessments Discharge Planning Best personality Fit: Professional and dedicated and friendly Strong communication skills Adaptable to different units.
