Job Description:
UCHealth, a leading nonprofit healthcare organization based in
Colorado, is seeking a dedicated Transplant Coord RN to join our
team. This role is pivotal in managing the comprehensive care for
organ transplant patients, from initial assessment through
post-operative recovery. The successful candidate will coordinate
with a team of healthcare professionals to ensure the delivery of
high-quality, patient-centered care across our hospitals and
outpatient facilities. As a Transplant Coord RN at UCHealth, you
will contribute to a culture of compassion and excellence in a
supportive and collaborative environment built on cutting-edge
medical practices and patient care innovations. Conduct initial
patient assessments and eligibility evaluations for organ
transplant. Coordinate with medical teams to plan and implement
comprehensive care plans. Monitor patient progress and adjust care
plans as necessary. Provide education and support to patients and
their families regarding transplant procedures and post-care
requirements. Participate in multidisciplinary team meetings to
discuss patient treatment and progress. Ensure compliance with
healthcare regulations and standards related to organ
transplantation. Manage documentation and patient records regarding
transplant processes. Excellent coordination and communication
skills Strong organizational and critical-thinking skills
Proficiency in managing multiple tasks and priorities Experience
with electronic health record systems Ability to work effectively
in a team-oriented environment Medical Insurance Dental Insurance
Vision Insurance Life Insurance 401k Valid Registered Nurse (RN)
license in the state of Colorado. Certification in Clinical
Transplant Coordination (CCTC) preferred.
